dragon naturally speaking seems to be widely used in the corporate world. Particularly for the sight impaired.

i had a play with vistas one and it works ok in notepad or other simple programs but in word it seems to select settings all the time.
dragon isnt cheap but it seems to have more accuracy due to better set up.
Not sure its worth the money though.
